Requirements
- Active, unencumbered Registered Nurse (RN) license in South Carolina or Georgia OR Compact (Multistate) RN license (NLC) with ability to practice in both states
- Minimum 2 years of nursing experience required
- Eligible to practice in assigned service areas across SC
- Valid driver’s license and reliable transportation
- Ability to meet health, background, and compliance requirements
Nice To Haves
- Experience in hospice, home health, long-term care, or case management preferred
Responsibilities
- Provide skilled nursing care to hospice patients in homes, assisted living, and long-term care settings
- Perform comprehensive assessments and develop individualized plans of care
- Support admissions, case management, and routine visits as needed
- Coordinate care with physicians, caregivers, and the interdisciplinary team
- Educate patients and families on disease progression, symptom management, and end-of-life care
- Provide compassionate presence and support at time of death
- Complete timely, accurate documentation in compliance with hospice regulations
- Supervise and guide hospice aides and caregivers
