Requirements

  • Active, unencumbered Registered Nurse (RN) license in South Carolina or Georgia OR Compact (Multistate) RN license (NLC) with ability to practice in both states
  • Minimum 2 years of nursing experience required
  • Eligible to practice in assigned service areas across SC
  • Valid driver’s license and reliable transportation
  • Ability to meet health, background, and compliance requirements

Nice To Haves

  • Experience in hospice, home health, long-term care, or case management preferred

Responsibilities

  • Provide skilled nursing care to hospice patients in homes, assisted living, and long-term care settings
  • Perform comprehensive assessments and develop individualized plans of care
  • Support admissions, case management, and routine visits as needed
  • Coordinate care with physicians, caregivers, and the interdisciplinary team
  • Educate patients and families on disease progression, symptom management, and end-of-life care
  • Provide compassionate presence and support at time of death
  • Complete timely, accurate documentation in compliance with hospice regulations
  • Supervise and guide hospice aides and caregivers
