About The Position

Join Southcoast Health, a not-for-profit, charitable, health system with multiple hospitals, clinics and facilities throughout Southeastern Massachusetts and Rhode Island. We are searching for a talented RN-Practice Navigator-Oncology to provide consistent care coordination across the continuum, assessing the physical, psychological and social needs of the patient. This role serves as an ongoing resource for patients during the initial diagnosis and throughout treatment and survivorship, organizing and facilitating clinical visit needs. The RN-Practice Navigator will assist patients and families by providing education, support, and information regarding available resources for cancer patients within the Cancer Center and the community, and support patient and families to ensure compliance with care.

Requirements

Strong oncology background required.
Bachelor of Science in Nursing (BSN), or equivalent experience required.
Basic knowledge of insurance reimbursement systems and financial hardships.
Critical thinking skills and interpersonal skills.
Ability to develop collaborative relationships both internally and externally.
Ability to multi-task, handle stressful situations in a fast-paced environment required.
Knowledge and clinical expertise in community services available to patients and their families required.
Over three years of clinical oncology experience in an outpatient or inpatient setting required.
Current R.N. licensure in Massachusetts.
Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.

Nice To Haves

Certification like OCN or willingness to obtain within 1 year of hire.
Specific training as an Oncology Patient Navigator preferred.
Cardiopulmonary Resuscitation (CPR) preferred.
Oncology Nurse Certification preferred; Achievement of Oncology Nurse Certification within first eighteen months of employment required.

Responsibilities

Provide consistent care coordination across the continuum, assessing the physical, psychological and social needs of the patient.
Serve as ongoing resource for patients during the initial diagnosis and throughout treatment and survivorship.
Organize and facilitate clinical visit needs.
Assist patients and families by providing education, support and information regarding available resources for cancer patients within the Cancer Center and the community.
Provide educational materials and referrals to appropriate support groups and services.
Support patient and families to ensure compliance with care.
