RN-Pediatric GI Nurse Coordinator

About The Position

The Nurse Coordinator is a Registered Nurse, reporting to Nursing leadership within the Pediatric Specialty Centers and under the direction and supervision of the Attending Physician for the specific specialty or program. The primary role of the nurse coordinator is to ensure seamless coordination of patient care accross the continuum. This includes clinical practice, care coordination, education, quality improvement, with significant latititude for exercising professional judgement. The Nurse Coordinator is an integral member of the multidisciplinary team providing developmentally approprioate care to patients and families within the specialty service.

Requirements

  • Registered Nurse
  • Connecticut RN license.
  • At least 2-3 years experience in a pediatric inpatient or outpatient setting, Including experience in the specialty area of the Nurse Coordinator role
  • Licensed as a Registered Nurse from the State of Connecticut.
  • Excellent interpersonal and organizational skills.
  • Ability to assert oneself, advocate for patients, and work independently with minimal supervision.
  • Ability to manage multiple priorities.
  • Computer experience and knowledge of word processing and data entry systems.
  • Must be able to communicate efffectively with all members of the healthcare team, patients and families.
  • Must value and execute "patient -centered model of care."
  • Must be flexible to meet the demands of the specialty, outpatient clinic session, specialty and patient/family needs

Nice To Haves

  • Bachelor's Degree in Nursing preferred
  • Relevant experience in the specialty area, pediatrics or in the Nurse Coordinator role preferred.

Responsibilities

  • The Pediatric Nurse Coordinator is the lead member of the health care team coordinating all aspects of patient care for the specific specialty.
  • Provides patient and family education on appropriate medical condition, including signs, symptoms , treatment regimens, side effects, outcomes expected, and potential adverse reactions or outcomes. Including manangement of condition, promoting optimal level of health.
  • Functions as a key member of the multi-disciplinary team coordinating all aspects of patient care across the continuum.
  • Participates in data collection related to clinical and operational efficiencies.
