About The Position
As our RN Patient Care Coordinator in Wheaton, IL, you’ll keep home health care moving smoothly—connecting the dots so patients get timely, compliant, high-quality services. You’ll be the clinical point person who keeps communication crisp and care plans on track.
Requirements
- Active Illinois RN license
- 1–2 years nursing experience
- Working knowledge of Medicare home health regs, OASIS, and care coordination
- EMR proficiency (WellSky/Kinnser)
Nice To Haves
- Home health experience preferred
Responsibilities
- Serving as the primary clinical liaison for field clinicians, physicians, referral sources, and patients/families
- Coordinating and communicating care needs, schedule updates, and follow-ups
- Supporting clinicians with clinical questions, orders clarification, and home health protocols
- Initiating and coordinating care plans aligned to physician orders and Medicare requirements
- Monitoring patient status and escalating changes to the interdisciplinary team
- Ensuring timely OASIS, visit notes, and documentation submission
- Assisting with case conferences, reassessments, and care plan updates
