About The Position

The Community Care RN provides and supervises professional, independent nursing care for Providence participants in the community. Care is based in the nursing process and is consistent with the Oregon Nurse Practice Act. Applies knowledge of assessing, planning, ordering, giving, delegating, teaching and supervising professional nursing care delivery that promotes patients' optimal health and independence. Providence PACE is a Program of All-Inclusive Care for the Elderly that strives to keep older adults as healthy as possible living in the community through clinics, home visits and more. Requirements

• Oregon Registered Nurse License
• National Provider BLS - American Heart Association
• 2 years RN experience in PACE, home health, skilled nursing facility, community health, hospice nursing or related experience.

Nice To Haves

• Bachelor’s Degree in Nursing
• Current recognition by a national professional organization in care management, geriatrics or hospice
