Requirements

  • Degree from accredited academic nursing program (If a program is under conditional status, approval may be granted by system CNO)
  • Minimum ADN preparation with expectation of BSN completion within 5 years of hire
  • BLS within 30 days of hire
  • State of New Mexico or Compact State Nursing License or Graduate Nurse Permit required
  • ACLS required
  • BLS required
  • PALS required

Responsibilities

  • Responsible for walk in and telephone triage.
  • Coordinates patients care through collaboration with the patient/family and healthcare team.
  • Provides patient care by assessing, planning, implementing, evaluating and documenting individual patient needs, actions taken and patient responses.
  • Responsible for case management based on PCP panels, in collaboration with network providers to include nurse visits, telephone management, etc.
  • Delegates or assigns aspects of patient care to healthcare team members commensurate with their validated competency to perform the task.
  • Documentation within scope of practice.
  • Performs other functions as required.
