The RN Orthopedic Service Line Coordinator will act as the primary point of contact for orthopedic patients from referral to recovery. This role involves coordinating appointments, diagnostics, pre-operative clearances, and post-operative follow-ups, ensuring timely movement of patients through the care continuum and minimizing delays. The coordinator will educate patients and families on procedures, expectations, recovery plans, and care pathways, while also identifying and addressing barriers to care such as transportation, financial, or social determinants of health. Additionally, the role supports the implementation and adherence to standardized orthopedic pathways, monitors patient progress, and facilitates prehabilitation and post-acute care coordination. Collaboration with orthopedic surgeons, advanced practice providers, and multidisciplinary teams is key, as is ensuring seamless care transitions through interdisciplinary communication. The coordinator will also serve as an advocate for patients and families, promote patient engagement tools, and monitor patient satisfaction. Operationally, the role involves tracking key service line metrics, identifying opportunities for process improvement, and participating in quality improvement initiatives. Accurate documentation in the electronic medical record and tracking of patient navigation activities and outcomes are also essential.
