About The Position

This role focuses on facilitating care coordination and discharge planning to ensure smoother and smarter care transitions. The position is PRN status and is located in scenic Northwest Montana, near Glacier National Park, ski resorts, and Flathead Lake. Logan Health Medical Center is seeking a PRN RN or Social Worker to join their Case Management Team. The role involves overseeing the transition of care from inpatient to outpatient settings, managing inpatient utilization, and collaborating with interdisciplinary teams to develop effective discharge plans.

Requirements

  • Current Montana RN license or a multi-state compact license with authorization to practice nursing within the state of Montana (required for RNs).
  • Master’s Degree or Bachelor's Degree in Social Work, or related Social Service Field (required for Social Workers).
  • Minimum of two (2) years’ RN experience in a similar position within a medical setting (e.g. physician office, clinic, or hospital) (required for RNs).
  • Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.
  • Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.
  • Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.
  • RQI Provider (BLS) within 7 days of hire/transfer or before independent patient care, whichever comes first (required for RNs).

Nice To Haves

  • BSN preferred (for RNs).
  • Minimum of one (1) year case management experience preferred (for RNs).
  • Experience in a medical or hospital setting, or discharge planning, highly preferred (for Social Workers).
  • Must have prior social services experience, preferably in a healthcare setting (for Social Workers).
  • Current CPR per assigned area(s) may be required.

Responsibilities

  • Facilitate Care Coordination and Discharge Planning, overseeing the transition of care from inpatient to outpatient settings, ensuring seamless coordination of services for optimal patient outcomes.
  • Manage and review inpatient services to ensure appropriate utilization of resources and adherence to established guidelines (Inpatient Utilization Management).
  • Actively participate in interdisciplinary team meetings to develop and implement effective discharge plans, fostering communication and collaboration across care teams.
  • Identify clinical and social needs of patients related to admission and/or discharge.
  • Formulate a plan for transition in a safe and timely manner to the least restrictive environment that can meet each patient’s needs.
  • Collaborate with stakeholders to address social determinants of health (SDOH) and a thorough admission and/or discharge plan as applicable to assigned area(s).
  • Identify patients at risk for readmission and work with the interdisciplinary team across the continuum to formulate and implement a discharge plan to decrease the risk for readmission.
  • Act as a resource to providers, patients, caregivers, and the interdisciplinary team regarding community resources and reimbursement as applicable to assigned area(s).
  • Exhibit knowledge and skills necessary to provide for the physical, psychological, and cultural needs appropriate to the age, ethnicity, and special needs of the patient.
  • Acknowledge and advocate for a patient’s right to self-determination and informed consent.
  • Consistently reflect ownership and professionalism towards Case Management.
  • Identify areas in need of improvement and serve an active role in developing solutions.

  • Identify and obtain necessary provider documentation and orders to ensure compliance with all applicable Logan Health, State, and Federal requirements as they relate to Utilization Review, Discharge Planning, and Care Coordination processes as applicable to assigned area(s).
  • Communicate frequently with patient and family/support to promote the development of a patient-focused plan, and ensure that the patient and family/support members are informed and involved in decisions about care.
  • Maintain regular and consistent attendance as scheduled by department leadership.
