About The Position

Monadnock Family Services is seeking compassionate, collaborative, and recovery-oriented healthcare professionals to join our outpatient psychiatric Health Services team serving children, adolescents, adults, and families throughout Cheshire County. The Health Services Nurse provides nursing support and health-related services within outpatient behavioral health programs, including Adult Services, Residential Support Services (RSS), and Children’s Outpatient Services. This role supports psychiatric and medical treatment coordination through nursing consultation, medication support, health education, collaboration with multidisciplinary treatment teams, and implementation of agency and regulatory standards. The Health Services Nurse works collaboratively with psychiatrists, psychiatric nurse practitioners, therapists, case managers, and support staff to promote integrated, person-centered care and positive client outcomes within a Community Mental Health Center (CMHC) environment.

Requirements

  • Current New Hampshire licensure as one of the following: Registered Nurse (RN) or Licensed Practical Nurse (LPN)
  • Minimum of one (1) year of experience in behavioral health, psychiatric services, medical services, or a related healthcare setting.
  • Strong interpersonal, communication, and organizational skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Basic proficiency with electronic medical record systems and Microsoft Office applications.

Nice To Haves

  • Certification in Psychiatric Mental Health Nursing.
  • Experience working in a Community Mental Health Center (CMHC) or outpatient psychiatric setting.
  • Experience working with children, adolescents, adults, and families in behavioral healthcare environments.
  • Knowledge of trauma-informed and recovery-oriented care practices.
  • Experience with medication administration and psychiatric medication education.

Responsibilities

  • Provide nursing support and health-related interventions to clients receiving outpatient behavioral health services.
  • Conduct individual nursing consultations within the scope of licensure and agency policy.
  • Provide client education regarding: Psychiatric and medical medications, Medication adherence and side effects, Physical and behavioral health conditions, Wellness, prevention, and self-management strategies.
  • Administer medications and support medication-related treatment needs in accordance with provider orders, agency policy, and regulatory standards.
  • Complete prescription renewals and medication coordination activities as directed by psychiatric providers.
  • Monitor and report medication concerns, side effects, adverse reactions, or clinical changes to appropriate providers and supervisors.
  • Assist in promoting continuity of care through communication and coordination with internal and external providers.
  • Provide guidance, support, and training to unlicensed staff regarding medication distribution and related procedures in accordance with agency policy and scope of practice.
  • Participate as an active member of Adult Services, Children’s Services, Residential Services, and Health Services treatment teams.
  • Collaborate with psychiatric providers, therapists, case managers, nursing staff, and leadership to support integrated service delivery.
  • Contribute to interdisciplinary treatment planning and care coordination efforts.
  • Assist with health-related education and training initiatives for clients and staff.
  • Maintain accurate, timely, and professional clinical documentation in accordance with agency policy, Medicaid requirements, professional standards, and regulatory expectations.
  • Adhere to HIPAA, confidentiality, infection control, medication management, and documentation standards.
  • Support compliance with OSHA standards, agency safety protocols, and medication administration procedures.
  • Participate in quality improvement activities, audits, and compliance initiatives as assigned.
  • Demonstrate professional conduct and ethical behavior consistent with the mission, values, and standards of Monadnock Family Services.
  • Attend required meetings, supervision sessions, trainings, and professional development activities.
  • Participate in annual learning plans and maintain all required licenses, certifications, and continuing education requirements.
  • Contribute positively to problem-solving, teamwork, and operational efficiency.
  • Perform additional duties within the scope of the role as assigned by the Nursing Services Manager and/or Chief Medical Officer.
