About The Position

Centrally located within the heart of California, Adventist Health Tulare has been serving the Central Valley since 2018. We are comprised of a 108-bed acute care hospital, emergency department and provide primary and specialty care services to best serve our community. Tulare is situated in a delightful and geographically diverse region. There are a variety of opportunities for all-season outdoor recreation such as camping, hiking, water and snow skiing, fishing and boating. It is also within an hour's travel time to Yosemite, Sequoia and Kings Canyon National Parks. In addition to the beautiful landscape, it also offers a great cost of living and close-knit communities. Job Summary: Maintains responsibility for the clinical and educational practices of qualifying patients and protocols. Supervises the care program. Performs daily rounds on inpatients, monitors patients' progress through continuum of care, and tracks key indicators to ensure The Joint Commission compliance for evidence-based practices for patient safety are met. Provides care coordination, care plan development and a skilled nursing assessment. Works with surgeons to ensure patients/families have an understanding of the patient’s procedure, condition and after surgery plan of care.

Requirements

  • Registered Nurse (RN) licensure in the state of practice: Required
  • Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification from approved vendor per AH policy: Required
  • Advanced Cardiovascular Life Support (ACLS OR HS-ACLS OR RQIACLS) certification from approved vendor per AH policy: Required
  • AHA-BLS or ARC-BLS: Required
  • AHA-ACLS or ARC-ALS: Required

Nice To Haves

  • Bachelor's Degree in Nursing (BSN): Preferred
  • Nursing experience: Preferred
  • Specialty Certification: Preferred

Responsibilities

  • Maintains responsibility for the clinical and educational practices of qualifying patients and protocols.
  • Supervises the care program.
  • Performs daily rounds on inpatients, monitors patients' progress through continuum of care, and tracks key indicators to ensure The Joint Commission compliance for evidence-based practices for patient safety are met.
  • Provides care coordination, care plan development and a skilled nursing assessment.
  • Works with surgeons to ensure patients/families have an understanding of the patient’s procedure, condition and after surgery plan of care.
  • Anticipates and assists with symptom management, including pain control.
  • Identifies opportunities to improve the patient/family experience of care and improve efficiency/effectiveness of resource use.
  • Participates in family support, teaching, discharge planning and continuity of care across sites of care, in collaboration with the primary care team.
  • Participates in both formal/informal education for all disciplines.
  • Maintains responsibility for forming/leading a multidisciplinary team that will meet monthly.
  • Acts as resource for educating frontline staff about updates/changes in service, or care, for the qualifying patient.
  • Maintains accountability for key performance measures.
  • Works with team to identify/track key performance measures, including but not limited to, patient satisfaction, physician satisfaction, length-of-stay and discharge disposition.
  • Works with the data mining support area to distribute reports to key stakeholders and participates in process improvement initiatives, as required.
  • Functions as member of an integrated professional team in the ongoing accurate assessment of patient’s needs through rounding with physicians/nurse caregivers to promote the development, implementation and evaluation of the nursing care plan on an ongoing basis.
  • Maintains responsibility for the development, revision and implementation for all patient education materials to educate patient/families on Specialty Care and services.
  • Incorporates an interdisciplinary approach, utilizes best practices and research findings.
  • Ensures all materials are standardized and consistent with standard order sets and clinical pathways.
  • Maintains accountability for working with physicians/staff, updating on an annual basis, or as needed, to maintain high quality of care standards.
  • Promotes evidence based practice by reading, interpreting and evaluating research for utilization in practice.
  • Provides support to the team for continual learning.
  • Participates in staff education on an individual and/or group basis, as needed.
  • Acts as a resource to all members of the health care team related to provision of care, enhancing assessment skills of others, establishing appropriate care standards and determining realistic goals for optimal patient outcome.
  • Maintains a current knowledge of changes in Specialty Care and Services and care management standards, and takes appropriate measures to incorporate into performance systems.
  • Performs other job-related duties as assigned.
