Centrally located within the heart of California, Adventist Health Tulare has been serving the Central Valley since 2018. We are comprised of a 108-bed acute care hospital, emergency department and provide primary and specialty care services to best serve our community. Job Summary: Maintains responsibility for the clinical and educational practices of qualifying patients and protocols. Supervises the care program. Performs daily rounds on inpatients, monitors patients' progress through continuum of care, and tracks key indicators to ensure The Joint Commission compliance for evidence-based practices for patient safety are met. Provides care coordination, care plan development and a skilled nursing assessment. Works with surgeons to ensure patients/families have an understanding of the patient’s procedure, condition and after surgery plan of care.
