This position involves caring for and treating patients as directed by a physician. The role requires receiving and reviewing diagnostic tests, performing and documenting vital signs, educating individuals on medication side effects, performing and recording AIMS testing, and completing/participating in agency assessments and treatment planning. The RN/Nurse may serve as the primary case holder, and the ACT nurse will fulfill case management duties. Responsibilities include observing medication setup, dispensing professional samples, ordering and coordinating injectable medications, administering injectable medications, obtaining prior authorizations, documenting and reporting significant symptoms or incidents, completing releases of information, and communicating with various healthcare professionals and guardians. The role also involves preparing for clinics, documenting in the electronic medical record, developing functional knowledge of agency systems and procedures, understanding and practicing Standard Precautions, participating in mandatory meetings, and responding to medical emergencies. The ACT Nurse must also participate in after-hours on-call duties. Other duties may be assigned, including providing medication training to AFC homes/Agency Staff and completing the nursing portion of OBRA assessments.
