Position Summary: Maintain the standards of nursing care and practice of the nursing department on the designated neighborhood. Plan, organize, implement and evaluate nursing services. Direct and supervise the work of nursing staff assigned and interface with the interdisciplinary team. Display visible leadership and guidance through change and disruptions.

Requirements

  • Graduate from a state-approved school of nursing with a license and current registration to practice in New York State required
  • Knowledge of nursing and medical practices and procedures, including laws, regulations and guidelines that pertain to long-term and transitional care philosophy
  • Leadership skills and willingness to work harmoniously with others
  • Excellent organizational and communication skills
  • Ability to interact with patients and families and care for their needs

  • Organizational skills: frequent interfacing with multiple disciplines, successfully overview patient intake and discharge procedures

Nice To Haves

Responsibilities

  • Maintain the standards of nursing care and practice of the nursing department on the designated neighborhood.
  • Plan, organize, implement and evaluate nursing services.
  • Direct and supervise the work of nursing staff assigned and interface with the interdisciplinary team.
  • Display visible leadership and guidance through change and disruptions.
  • Assure the delivery of comprehensive nursing services to meet a defined resident population needs in conformance with governmental regulations and St. John’s philosophy.
  • Interface with other departments and services to facilitate the delivery of patient care and ensure the participation of nursing in comprehensive care reviews with interdisciplinary team.
  • Demonstrate a working comprehension of insurance coverage and reimbursement and how it affects involved departments.
  • Review satisfaction survey results. Act as team leader for improving quality and customer service for residents from a nursing perspective.
  • Remain current in knowledge of nursing care requirements for all patients in the neighborhood.
  • Ensure maintenance of all nursing records in compliance with standards.
  • Participate in Nursing Service Quality Assurance Programs. Tend to patient care process and outcome, and implement QA activities on neighborhood.
  • Report infections and staff’s implementation of infection control policies and procedures.
  • Assume responsibility for continuity of patient care by assuring the assessment of health needs and maintenance of written individualized nursing care plan in coordination with other disciplines as needed.
  • Implement established medication systems and ensure the safety and completeness in medication and treatment administration.
  • Act as liaison between neighborhood staff and administration.
  • Attend nursing department and inter-departmental meetings and disseminate the appropriate information to staff. Represent the nursing department through committees.
  • Be an active participant in the Eden journey.
  • Help with other duties as needed that support the Core Team to fulfill St John’s mission vision and values.
