Alignment Health is seeking a compassionate and experienced Telephonic RN Case Manager for their Special Needs Plan (SNP) members. This is a fully remote, phone-based position where the nurse will help members navigate their care journeys, close gaps in care, and overcome barriers to better health. The role requires a California RN license and involves providing telephonic case management to medically complex and chronically ill members. The nurse will conduct comprehensive health assessments, create individualized care plans, coordinate care with internal and external partners, educate members and caregivers, monitor progress, and identify/resolve service or access issues.
