About The Position

This role integrates health care for patients/clients across the continuum of care. The RN Navigator collaborates with the healthcare team and other professionals to coordinate and ensure timely sequencing of care. This integration enhances patient flow and interdisciplinary communication, promoting early intervention. The role supports patients/clients and other family members by removing barriers to care. The position seeks to evaluate outcomes based on an integration among established clinical, financial, and utilization data. The RN Navigator functions in structured and unstructured health care settings, described as a geographic and/or situational environment that may not have established policies and procedures. They utilize independent nursing judgment when integrating health care. Knowledge of and utilization of appropriate age-related patient care protocols relating to the physical and psychological needs of pediatric, adolescent, adult, and geriatric patients according to the department scope of service is essential. The role may serve as a key link between providers, clinics, insurance concerns, communication, inpatient floors, and outside facilities. Establishing and maintaining good interpersonal relationships with those with whom there is contact is important. The ability to work with the growth and development needs of client populations in the clinical area is required. Strong communication skills are necessary. The role involves coordinating, facilitating, and negotiating with others. The RN Navigator monitors, evaluates, and trends patient responses utilizing structured care methodologies and works closely with ancillary and support services. An ability to solve problems and identify and implement process improvements when needed is crucial. The position requires self-motivation and the ability to work with little direction. The ability to facilitate groups, multidisciplinary teams, and implement best practices when necessary is also important. The RN Navigator functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in the state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.

Requirements

  • Graduate from an accredited nursing program, including, but not limited to, those accredited by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
  • Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Obtains and subsequently maintains required department specific competencies and certifications.

Nice To Haves

  • Bachelor's degree in Nursing strongly preferred in Bemidji, Marshfield, and Rapid City.
  • Required in Fargo and Sioux Falls (Sanford USD Medical Center and Clinics).
  • Required in Bismarck or enrolled and starting a Bachelor’s degree program within one year of hire with a completion date within three years upon date of hire into position.
  • Required in Fargo Network and Sioux Falls Network or employees hired after January, 2025, who do not possess a Bachelor’s degree in the required field, leadership may consider an educational plan with proven continuous action toward achieving a Bachelor’s degree in Nursing within five years of hire into position.
  • Approval from the Nursing Executive for the market is required in order to waive the bachelor degree requirement (at time of hire).
  • Additionally, for Rural Health Network facilities, for current employees, leadership may have considered acceptable qualifications and work experience equivalency based on facility size, rural market, and business need, prior to new Sanford education requirements.
  • Minimum of two years’ clinical experience preferred.
