About The Position

St. Elizabeth Healthcare is hiring a Registered Nurse (RN) Navigator for our Structural Heart department located within the Edgewood Hospital. The Heart & Vascular Navigator will function as a central figure in the overall operations of the Disease Specific Program/Center (i.e. Valve Center, Heart Failure Center, Prevention and Wellness, Chest Pain). The Navigator is a Registered Nurse who works closely with the department directors, managers and staff who have direct or indirect responsibilities for the program. Coordinates daily operations of the program/center including effective communication and collaboration of program activities with physicians and the multi-disciplinary team. Expected roles include direct patient contact and coordination of care ensuring optimal care and patient experience as well as efficiency for patients with cardiovascular disease (i.e. Valve disease, Heart Failure, Prevention and Wellness, Chest Pain). Demonstrates respect, dignity, kindness and empathy in each encounter with all patients, families, visitors and other employees regardless of cultural background.

Requirements

  • Bachelors Degree from an accredited nursing program.
  • If eligible, must have a Compact (multi-state) License in their primary state of residence.
  • Currently licensed to practice nursing in the state where work is being performed.
  • Obtains required CEU’s, including all state required courses.
  • Extensive knowledge of nursing standards and practice in their clinical area.
  • Excellent oral and written communication skills and an accomplished facilitator.
  • BLS (Basic Life Support)
  • ACLS (Advanced Cardiac Life Support) = Chest Pain Coordinator only
  • Five years of experience in Nursing or a related field.

Responsibilities

  • Coordinates the referral process and introduction of patients into the Heart & Vascular Center/program for purposes of screening, evaluation and assessment.
  • Guides and directs the continuum of care for patients from admission through discharge or screening through follow-up (including post discharge care and follow up as appropriate for this patient population).
  • Actively participates in the National Registry (i.e. TVT) and Disease Specific Center/Program registry to assure data related to patients is reported accurately and appropriately for purposes of Quality improvement.
  • Functions as a liaison between the program, vendors, physicians and multidisciplinary team members.
  • Develops and implements protocols for care of patients which are evidence based in collaboration with the directors, managers and staff of departments caring for the program specific patients.
  • Assists in the development and achievement of disease specific goals to support the overall performance of the service line
  • Develops and implements the educational materials and support needed by the patient and family and assures patients and families receive appropriate educational counseling to assist them in physical and psychosocial progress towards outcomes. This includes educational programs in the community.
  • Communicates effectively, both verbally and in written format and identifies areas for opportunity, growth and improvement to the medical and administrative team on an ongoing basis.
  • Collaborates with the Marketing Department to support program growth.
  • Participates in performance improvement/research activities designed to improve patient care and/or organizational processes.
  • Performs other duties as assigned.
