About The Position

This position involves caring for pediatric patients with multiple chronic health conditions, acute episodes of care, and behavioral health challenges, while promoting prevention and wellness. The role provides evidence-based, high-quality nursing care via telephone and MyChart, assists with care coordination and clinic procedures, and collaborates with a multidisciplinary team for positive patient outcomes. The RN Navigator will also provide education and guidance to patients and families, utilizing evidence and science to guide practice.

Requirements

  • Bachelor's Degree in Nursing (BSN) Required
  • 3 years of relevant clinical nursing experience Required
  • Registered Nurse license in the state of Wisconsin Upon Hire Required
  • Basic Life Support/CPR Upon Hire Required

Nice To Haves

  • Master's Degree in Nursing, Social Work, Public Health, Healthcare Administration, or related field Preferred
  • 1 year of relevant experience in the specific patient population being supported Preferred
  • Applicable Oncology certification based on patient population Preferred
