The RN Navigator - Pediatric Cardiovascular Surgery Clinic role is responsible for providing continuity of care by ensuring smooth transitions between care settings. This position develops a relationship with the patient and their multidisciplinary team to facilitate and/or navigate through subsequent treatment and follow-up. The role involves completing nursing assessments to identify individualized patient needs, collaborating with multidisciplinary teams to develop care plans, and supporting patients and families by providing education, addressing psychosocial needs, advocating, and identifying barriers to care. The RN Navigator guides patients and family members throughout the diagnosis, treatment, and follow-up process across the continuum of care. They provide expert clinical advice to the multidisciplinary team, design collaborative teaching plans, and plan/provide patient/family education and discharge planning, including wellness and health maintenance within the specialty. This role also coordinates timely, seamless, evidence-based care, evaluates and selects patient education materials, and provides education and outreach programs to patients, Advocate staff, and the community. Additionally, the RN Navigator assists leadership in developing program vision, goals, and objectives, and in the development, implementation, and maintenance of quality programs focused on best practice patient care outcomes, quality of life, and enhanced patient loyalty. The position requires the ability to demonstrate knowledge and skills appropriate for the age of the patients served, understanding growth and development principles and assessing patient status to meet age-specific needs.
