The position requires continuous review and evaluation of patient progress, communication of the plan of care, and assistance with resolving financial, psychosocial, functional, and administrative issues. The navigator provides emotional support, counseling, clinical education, and expert guidance to empower patients and families in their healthcare journey and decision-making.
