The RN Navigator - Oncology is accountable for the delivery of quality care to patients and families according to the individualized needs of the patient. This role involves providing appropriate care specific to the age of the customer/patient, performing initial and ongoing assessments, developing and revising plans of care, implementing interventions, performing technical skills competently, maintaining professional behavior, initiating actions to correct or minimize risks, and being fiscally accountable. The navigator guides patients and families toward informed decision-making by collaborating with a multi-disciplinary team, facilitating planning and education about diagnosis and treatment options, supporting patients through treatment, participating in treatment planning conferences, referring patients to support programs, teaching about prevention and early detection, facilitating education for clinical research trials, and assisting with the development of an ongoing survivorship plan.
