The Transition Care Management (TCM) RN Navigator, Ambulatory position will assist with quality reviews for set diagnoses that have a positive impact on CMS Star Ratings. This role integrates healthcare for patients/clients across the continuum of care. The RN Navigator collaborates with the healthcare team and other professionals to coordinate and ensure timely sequencing of care. This integration enhances patient flow and interdisciplinary communication, promoting early intervention. The role supports patients/clients and their family members by removing barriers to care and seeks to evaluate outcomes based on an integration among established clinical, financial, and utilization data. The position functions in structured and unstructured healthcare settings, described as a geographic and/or situational environment that may not have established policies and procedures. The RN Navigator utilizes independent nursing judgment when integrating health care and applies knowledge of appropriate age-related patient care protocols relating to the physical and psychological needs of pediatric, adolescent, adult, and geriatric patients according to the department's scope of service. This role may serve as a key link between providers, clinics, insurance concerns, communication, inpatient floors, and outside facilities. The position requires establishing and maintaining good interpersonal relationships, the ability to work with growth and development needs of client populations, and strong communication skills. The RN Navigator coordinates, facilitates, and negotiates with others, monitors, evaluates, and trends patient responses utilizing structured care methodologies, and works closely with ancillary and support services. The ability to solve problems, identify and implement process improvements, and function as a self-motivated individual working with little direction is essential. The role also involves facilitating groups, multidisciplinary teams, and implementing best practices when necessary. The RN Navigator functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in the state of practice and licensure, adhering to the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association.
