About The Position

LCMC Health is seeking an RN Navigator-ENT to coordinate communication regarding the medical treatment plan between patients, families, physicians, and other healthcare team members. This role involves communicating and coordinating the treatment plan with internal and external departments and agencies to ensure continuity of patient care. The RN Navigator will assist patients in understanding their diagnosis, treatment options, and available resources, and review medical documentation to support physicians in accurate treatment plan documentation. They serve as a crucial link between patients and all care providers, facilitating appointments for consults and support services, and following patients throughout the care continuum, including inpatient admissions. The role also involves collaborating with inpatient care management, working with service line personnel to collect data and track outcomes, and supporting strategic planning. The RN Navigator demonstrates expertise in assessing, planning, implementing, and evaluating patient care according to the nursing process, and assists patients and families in navigating the healthcare system to resolve treatment-related problems. They provide education on disease processes, treatment side effects, and community resources, and support patients during difficult decision-making periods. The position requires functioning in an organized and time-conscious manner, and may involve policy review and updates. Additionally, the RN Navigator serves as a resource for community educational events and staff education.

Requirements

  • 2 years of professional nursing experience in specialty area.
  • Louisiana State Board of Nursing - Registered Nurse License
  • American Heart Association - Basic Life Support (BLS) Certification

  • Bachelor’s degree in Nursing (BSN) or must be able to obtain a BSN within three years of their date of hire.

Responsibilities

  • Coordinates written and verbal communication relating to the medical treatment plan between the patient, family, physician, physician staff, and other team members.
  • Communicates and coordinates the treatment plan to internal and external departments and agencies, as needed, to establish and maintain the continuity of patient care.
  • Assists patients in understanding their diagnosis, treatment options, and the resources available.
  • Reviews the medical documentation to assist the physicians in obtaining complete and accurate documentation of their medical treatment plan.
  • Serves as an essential link between patients and all other care providers.
  • Facilitates appointments for consults and support services within established service standards, including social workers, PT, OT, ST, dietitians, physicians, financial resources, etc.
  • Follow patients throughout the care continuum, including inpatient admissions and collaborate with inpatient care management resources.
  • Work with service line personnel to collect data, track outcomes, and support strategic planning process.
  • Demonstrates the knowledge, skill, and coordination to provide nursing care and guidance to the patient from screening to survivorship.
  • Systematically and continually performs the functions of assessing, planning, implementing, and evaluating the care according to the nursing process.
  • Assists patients and their family members "navigate" the system by helping them find solutions to treatment-related problems.
  • Provides education and information to the patient and family, helping to make the care seamless, continuous, and comprehensive.
  • Initiates and documents patient teaching including family and significant others based on assessment of needs.
  • Uses the appropriate patient Education documentation modality regarding the disease process, expected side effects of treatment and community resources.
  • Supports the patient during difficult decision-making periods.
  • Functions in an organized and time-conscious manner.
  • May initiate and perform ongoing review of policies related to service provided.
  • Where appropriate, updates or writes new policies to enhance professional practice.
  • Serves as a resource for community educational events such as health fairs, screenings, symposiums, and lectures as well as staff education.
