About The Position

The RN multiple locations site manager is a registered professional nurse who ensures quality of patient care in the home. Primary function is for the overall supervision of patient care and monitoring of appropriate staffing and productivity. Coordinates care with the interdisciplinary group, patient, family and referring Hospice. Facilitates communication between the multiple locations site and the Parent office. Oversees clinical operations for Tomball and College Station.

Requirements

  • Graduate from an accredited diploma, associate, or baccalaureate nursing program.
  • Active, unencumbered state license as a registered nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state.
  • Current driver’s license.
  • One year of experience as a registered nurse in a clinical care setting.
  • Reliable transportation and valid auto liability insurance.
  • Must read, write, and comprehend English.

Nice To Haves

  • Home health experience preferred.

Responsibilities

  • Oversee clinical operations for Tomball and College Station
  • Directs and coordinates patient care
  • Assumes responsibility for continuity, quality, and safety of services delivered in compliance with State and federal regulations.
  • Participate in activities relevant to professional services including development of Hospice personnel.
  • Supervises and provides direction to subordinates, in an effort to ensure quality, compliance with Plan of Care and assessment and reassessment of patient’s needs and continuity of services by appropriate health care personnel.
  • Monitors QAPI Program and assures appropriate corrective measures are performed.
  • Promotes compliance with all fiscal intermediary and/or other third-party payors.
  • Monitors systems and identifies problem areas to DON, both verbally and through written reports.
  • Meets mandatory continuing education requirements of the Hospice/licensing board.
  • Promotes and educates regarding the concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
  • Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes
  • Provide patient care when needed and participate in after hours on call for all locations.
