About The Position

Jefferson Community Health & Life is looking for an RN to work in patient services. This Registered Nurse (RN) - Medical Clinic Nurse works in Patient Services and, as a Triage Nurse, provides professional nursing care through the assessment, triage, and coordination of patient needs via telephone, electronic communications, and in-person interactions. The RN utilizes clinical judgment to assess symptoms, prioritize patient needs, provide evidence-based nursing advice, and facilitate timely access to care. This role supports providers by coordinating patient care, implementing provider-approved standing orders and protocols, assisting with chronic disease management, and ensuring continuity of care. The RN Triage Nurse promotes safe, timely, and patient-centered care while functioning within the scope of practice for a Registered Nurse in the State of Nebraska and in accordance with organizational policies and regulatory requirements.

Requirements

  • Current Registered Nurse (RN) license in the State of Nebraska is required.
  • Current Basic Life Support (BLS) certification required.
  • Demonstrated knowledge of nursing assessment, clinical decision-making, and patient triage principles.
  • Strong critical thinking, problem-solving, and prioritization skills with the ability to assess patient needs and determine appropriate levels of care.
  • Excellent verbal and written communication skills with the ability to effectively interact with patients, families, providers, and healthcare team members.
  • Strong organizational and time management skills with the ability to manage multiple priorities in a fast-paced environment.
  • Proficiency in electronic health records (EHR) and other healthcare technology systems.
  • Ability to maintain patient confidentiality and comply with HIPAA and organizational policies.
  • Ability to work independently while collaborating effectively as a member of the interdisciplinary healthcare team.

Nice To Haves

  • Advanced Cardiac Life Support (ACLS) certification preferred.
  • Minimum of two (2) years of clinical nursing experience preferred, with experience in ambulatory care, family practice, emergency services, urgent care, or telephone triage strongly preferred.
  • Knowledge of chronic disease management, preventive care, and patient education principles preferred.

Responsibilities

  • Receives and manages incoming patient calls, assessing symptoms, urgency of needs, and appropriate levels of care.
  • Utilizes clinical judgment, evidence-based practice, and approved triage protocols to provide patient guidance and disposition recommendations.
  • Prioritizes patient concerns and facilitates timely access to care.
  • Documents all patient interactions, assessments, interventions, and recommendations accurately in the electronic health record (EHR).
  • Utilizes the nursing process to assess, plan, implement, and evaluate patient care according to the Registered Nurse scope of practice.
  • Processes prescription refill requests in accordance with clinic policies, provider-approved protocols, and regulatory requirements.
  • Reviews patient records, laboratory results, medication history, and recent visits prior to refill authorization.
  • Coordinates with providers regarding medication concerns, refill approvals, and medications requiring additional evaluation.
  • Educates patients regarding medication use, side effects, and adherence to prescribed treatment plans.
  • Completes prior authorization requests for medications, treatments, and services as required by insurance plans.
  • Communicates effectively with pharmacies, insurance companies, providers, and patients to facilitate timely processing.
  • Notifies patients of authorization approvals, denials, and alternative treatment options as directed by the provider.
  • Perform nurse visits, blood pressure checks, and other clinical services as appropriate.
  • Administers medications, injections, immunizations, and treatments according to provider orders, standing orders, and clinic policies.
  • Assesses walk-in patients and determines the appropriate care pathway based on clinical presentation and established protocols.
  • Identify abnormal findings or changes in patient condition and promptly reports concerns to the provider.
  • Demonstrates proficient clinical skills and maintains competency in required nursing procedures and equipment.
  • Coordinates patient care activities with providers, specialists, pharmacies, ancillary departments, and community resources.
  • Provides patient and family education regarding diagnoses, medications, treatment plans, preventive care, and follow-up instructions.
  • Encourages patient engagement, self-management, and adherence to treatment recommendations.
  • Facilitates effective communication between patients, families, providers, and members of the healthcare team.
  • Collaborates effectively with providers and interdisciplinary team members regarding patient needs, care plans, and outcomes.
  • Communicates professionally and respectfully with patients, families, coworkers, and external healthcare partners.
  • Participates in care coordination efforts to promote continuity of care and positive patient outcomes.
  • Informs and involves leadership regarding patient care concerns and operational issues in a timely manner.
  • Promotes patient safety through adherence to infection prevention standards, medication administration policies, and patient identification procedures.
  • Performs nursing care, treatments, and procedures safely and within the RN scope of practice.
  • Utilizes appropriate body mechanics, transfer techniques, and safety practices to reduce risk of injury to patients and staff.
  • Maintains a clean, organized, and safe clinical environment.
  • Participates in departmental meetings, training programs, and continuing education activities.
  • Meets annual competency and educational requirements.
  • Assists in the orientation and training of new employees as assigned.
  • Participates in quality improvement, patient satisfaction, and performance improvement initiatives.
  • Identifies opportunities for process improvement and contributes to solutions that enhance patient care and operational efficiency.
  • Serves as a positive role model by demonstrating professionalism, accountability, integrity, teamwork, and respect for others.
  • Comply with all federal, state, and local regulations governing nursing practice and healthcare operations.
