The Med Hub Nurse serves as the central clinical liaison between the patient, Emergency Department providers, admitting providers, the Transfer Center, and regional hospitals. This role is responsible for coordinating the admission and transfer process for appropriate low-acuity patients, ensuring efficient communication and seamless transitions of care across the health system. This position requires a caregiver who is empathetic, understanding and nurturing to ensure quality patient care and effective life-saving treatment. Requirements
- Graduate from an accredited School of Professional Nursing (ADN or BSN program)
- Current state licensure as a Registered Nurse (RN)
- One year of current acute care RN experience OR military RN experience
- Basic Life Support (BLS) certification through the American Heart Association (AHA) OR American Red Cross
- Advanced Cardiac Life Support (ACLS) certification through the American Heart Association (AHA) OR American Red Cross
- Emergency Nurse Pediatric Course (ENPC - pediatric) upon hire or within 24 months of beginning work in the Emergency Department.
- Pediatric Advanced Life Support (PALS) through the American Heart Association (AHA) OR American Red Cross within one year of hire

Responsibilities
- Provide direct nursing care.
- Implement and monitor patient care plans.
- Monitor, record and communicate patients’ conditions.
- Evaluate, update and revise plan of care to facilitate achievement of planned and expected outcomes
- Note and carry out physician and nursing orders.
- Assess and coordinate patients’ discharge needs.
- Educate patients and family with educational materials considering cultural issues and other special concerns
- Administer prescribed medications and treatment in accordance with approved nursing protocol
