Requirements

  • Current RN license in the State of Maryland
  • MDS certification or proven experience completing MDS assessments
  • Minimum 2 years of clinical experience. Minimum of 1 year of experience in a long-term care setting required
  • Must possess strong written and oral communication skills; able to maintain confidential information
  • Must be proficient in Microsoft Office
  • Strong organizational and analytical skills with excellent attention to detail.
  • A collaborative spirit and a dedication to mission-driven service in a senior care community

Nice To Haves

  • Prior experience as an MDS coordinator is strongly preferred

Responsibilities

  • Demonstrate commitment to NLCS’ core values in all aspects of your work
  • Support NLCS policies, procedures, guidelines, and code of conduct standards

  • Foster a positive work environment that promotes team member engagement and satisfaction
  • Coordinate individualized plans of care using an interdisciplinary team approach
  • Review and evaluate resident MDS assessments and care plans to ensure proper implementation
  • Participate in resident care plan conferences
  • Maintain the MDS schedule within required time frames and communicate pending assessments to relevant directors
  • Generate and distribute the monthly MDS calendar
  • Complete and audit MDS assessments, ensuring all required signatures are obtained
  • Transmit electronic copies of assessments to the Centers for Medicare and Medicaid Services (CMS) and reconcile all transmissions
