Requirements

Current RN license in the State of Maryland
MDS certification or proven experience completing MDS assessments
Minimum 2 years of clinical experience. Minimum of 1 year of experience in a long-term care setting required
Must possess strong written and oral communication skills; able to maintain confidential information
Must be proficient in Microsoft Office
Strong organizational and analytical skills with excellent attention to detail.
A collaborative spirit and a dedication to mission-driven service in a senior care community

Prior experience as an MDS coordinator is strongly preferred

Responsibilities

Coordinate individualized plans of care using an interdisciplinary team approach
Review and evaluate resident MDS assessments and care plans to ensure proper implementation
Participate in resident care plan conferences
Maintain the MDS schedule within required time frames and communicate pending assessments to relevant directors
Generate and distribute the monthly MDS calendar
Complete and audit MDS assessments, ensuring all required signatures are obtained
Transmit electronic copies of assessments to the Centers for Medicare and Medicaid Services (CMS) and reconcile all transmissions
