About The Position

Aviata at Emerald Shores is seeking a skillful and compassionate RN or LPN MDS Coordinator to join their team. The role involves overseeing the coordination and completion of the Resident Assessment Instrument (MDS, CAA’s and Care Plan) in accordance with current Federal and State Regulations. The coordinator will also monitor and document the management of Medicare and Managed Care residents in collaboration with facility team members, and contribute to innovative healthcare by implementing new ideas and concepts to improve systems and processes.

Requirements

• Must possess a current, unencumbered, active state license to practice as an RN.
• Six (6) months of experience as a MDS Coordinator.
• You must be qualified, compassionate, and dedicated to a job well done.

Nice To Haves

• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• RAC-CT or RNAC preferred.

Responsibilities
