Requirements

Graduate of an accredited school of nursing with a current RN license in the state of employment
MDS 3.0 and case management experience in a skilled nursing or post-acute care setting required
Ability to work independently while collaborating effectively with interdisciplinary teams
Strong organizational, communication, and critical thinking skills
Working knowledge of Microsoft Office applications

Nice To Haves

Previous long-term care or skilled nursing experience
RAC-CT certification or willingness to obtain within six months of hire and maintain certification requirements

Responsibilities

Coordinate the Resident Assessment Instrument (RAI) process to ensure accurate and timely completion of resident assessments
Ensure compliance with Medicare, Medicaid, OBRA, and other payer program requirements
Ensure MDS assessments accurately reflect each resident's physical, mental, and psychosocial status
Verify documentation supports services provided and reimbursement accuracy
Support interdisciplinary collaboration for accurate care planning, assessment completion, and ongoing case management coordination
Collaborate with clinical and administrative teams regarding admissions, transitions of care, and payer communication related to skilled nursing services
Monitor assessment schedules and ensure regulatory timelines are met
Assist with quality improvement initiatives and survey readiness efforts
