This position is responsible for conducting and coordinating the completion of the required Resident Assessment Instrument (RAI) and for the implementation and ongoing evaluation of each resident’s comprehensive plan of care. The role involves accurate completion of all MDS assessments and supporting documentation, scheduling and leading interdisciplinary care plan meetings, and auditing medical records. The MDS Coordinator also collaborates with health plans and physicians to ensure insurance approvals and eligibility, and acts as a primary resource for problem-solving related to the SNF prospective payment system and quality reporting program.
