RN Manager - Home Health
About The Position
Supports the Home Health East Portland team by providing oversight of all patient care services and personnel, including making patient and personnel assignments, coordinating patient care and referrals, ensuring continuous assessment of patient needs, and overseeing the development, implementation, and updates of individualized care plans. This role also involves reviewing required documentation and facilitating communication between field staff, office staff, patients, and providers. Requirements
- Bachelor’s Degree in respective Clinical Specialty.
- National Provider BLS - American Heart Association within 30 days of hire.
- Oregon Registered Nurse License upon hire.
- 1 year of supervisory/leadership experience.
Nice To Haves
- EMR experience (EPIC, Homecare Homebase)
- 3 years of Home Health Experience.
- 3 years of Supervisory Experience
Responsibilities
