About The Position

We are seeking a dynamic and experienced Charge Nurse to lead our 6A-2P shift, ensuring exceptional patient care and operational excellence within a fast-paced healthcare environment. As a Charge Nurse, you will serve as a pivotal team leader, coordinating nursing activities, supervising staff, and maintaining high standards of clinical practice. Requirements

  • Valid LVN or RN license with current Texas state licensure; additional certifications such CPR required
  • Extensive clinical experience in skilled nursing home setting required.
  • Proven leadership skills with ability to coordinate teams effectively in high-pressure environments.
  • Proficiency in electronic health records (EHR) systems including Point Click Care; familiarity with ICD-10/ICD-9 coding for accurate documentation.
  • Strong knowledge of physiology, anatomy, pharmacology, and medical terminology; experience with specialized areas such as dialysis, ventilator management, or IV infusion therapy is advantageous.
  • Excellent patient assessment skills with the ability to perform physical exams and interpret diagnostic evaluations accurately.
  • Knowledge of infection control protocols and sterile processing procedures; ability to manage complex cases involving dementia care or behavioral health needs.
  • Commitment to maintaining HIPAA compliance and safeguarding patient confidentiality at all times.

Nice To Haves

Responsibilities

  • Provide direct supervision and support to staff during the 6A-2P shift, ensuring adherence to clinical protocols and safety standards.
  • Coordinate patient care activities across units, including assessments, medication administration, and documentation using electronic health record (EHR) systems such as Epic or Cerner.
  • Oversee patient admissions, discharges, and transfers while ensuring compliance with hospital policies and regulatory requirements like HIPAA.
  • Collaborate with interdisciplinary teams—including physicians, therapists, case managers, and support staff—to develop and implement individualized care plans.
  • Monitor patient vital signs, conduct physical examinations when necessary, and respond promptly to emergent situations such as airway management or acute pain episodes.
  • Lead incident investigations, complete detailed incident reports, and participate in quality improvement initiatives aligned with NCQA standards.
  • Manage staffing schedules, supervise team members’ performance, provide mentorship for new nurses, and facilitate ongoing training on topics like infection control and aseptic technique.
