About The Position

Capitol View Transitional Care Center is a state-of-the-art true transitional care unit (TCU) located in Regions Hospital. We provide quality care to patients 18+ years of age who are in need of a short-term stay, with an average length of stay of 17 days. This role offers a rewarding and fun work environment in a TCU setting, with a patient to nursing staff ratio three times lower than the average nursing home.

Requirements

• Graduate of an accredited school of nursing
• Currently licensed as a LPN or RN with the Minnesota Board of Nursing
• Current CPR Card Certification
• Must be able to attend and have reliable transportation to and from HealthPartners' Nursing Orientation in Bloomington on Tuesday and Wednesday from 8am-4:30pm.

Responsibilities

• Provide direct care to patients by providing ADLs, accessing physical and mental status of patient to determine efficacy of treatment and care plan, administer medication, and complete all documentation.
• Prepare room preparation and room discharge of patients.
• Answer call lights and provide follow-up to ensure needs are met.
• Answer questions from patients and families or direct them to the appropriate person.
• Performs other duties as assigned.
