About The Position

Luther Park Community is seeking an RN/LPN for the Trinity Center for a FT 10p-6:30a EOW shift. The Trinity Center is a 120-bed skilled nursing facility which specializes in Alzheimer’s care. Requirements

• Graduate of an approved school of nursing for RN/LPN
• Current nursing license in good standing in the state of Iowa
• Knowledgeable with current changes in nursing practice
• Excellent written and oral communication skills
• Background check, physical, drug screen and TB Test required

Nice To Haves

• Current CPR certification preferred
• One year of experience in a geriatric setting preferred

Responsibilities

• Planning, organizing, and carrying out nursing care of residents.
• Orienting, instructing, and supervising other personnel: CNAs, CMAs.
• Complying with established policies and procedures and performing professional nursing duties within scope of licensure.
• Keeping DON/ADON informed of resident status and other pertinent matters.
• Admitting and orienting new residents and involving family with orientation.
• Checking physician’s orders, transfer forms and pre-admission information.
• Reviewing resident’s medical and social history.
• Completing admission and documentation forms.
• Ensuring resident's supplies and medications are ordered and equipment is working properly.
• Maintaining proper documentation of facility records, i.e. narcotic records, medical chart forms, weekly skin assessments.
• Maintaining daily, weekly, or monthly weight and vital signs, and informing physician/DON/ADON and On Call Manager of any change.
