About The Position

The RN/LPN is responsible for assisting the ADC Director with management responsibilities including monitoring health needs of participants. This position will also be responsible for expanding center activities and developing resources to motivate program participants. The RN/LPN will also be responsible for revising Care Plans and Assessments/re-assessments and working with the case manager on admitting and re-admitting new clients.

Requirements

  • RN or LPN licensure required.
  • Must participate and maintain CPR, First Aid certifications and keep CEU’s up to date.
  • Must have a valid driver’s license and the flexibility and willingness to change schedules and duties as needed according to staff availability.
  • Must be willing to travel the service area as needed.
  • TB test is required.
  • Agency policy regarding drug and alcohol testing and criminal background checks applies to this position.
  • Must maintain a high level of professionalism, demonstrated by a positive attitude, ability to function as a team member, and a commitment to the overall mission of the agency.

Nice To Haves

  • Prior experience working with the elderly and health needs associated with the elderly strongly preferred.
  • Experience with dispensing medication, familiarity with Care Plans and Assessments also preferred.

Responsibilities

  • Assist with the overall management and operations of the Active Day Center.
  • Monitor the health needs of participants, including vital signs, symptoms, and overall well-being.
  • Administer medications as prescribed and accurately document medication administration.
  • Collaborate with the assigned case manager from the CHFS (Cabinet for Health and Family Services) on Care Plans and Assessments/re-assessments for participants.
  • Assist in admitting and re-admitting new clients, ensuring all necessary paperwork and assessments are completed.
  • Prepare and assist participants with snacks and meals, including opening containers, cutting up food, and feeding participants as needed.
  • Ensure dietary restrictions and preferences are considered.
  • Lead and facilitate exercise sessions, games, crafts, and special activities.
  • Develop innovative resources to motivate and engage participants in program activities.
  • Assist participants on and off the bus, ensuring their safety and comfort during transportation.
  • Transfer participants from chairs to wheelchairs using proper lifting and transferring techniques.
  • Assist participants with toileting needs and personal hygiene.
  • Maintain certification in CPR, First Aid, and other required trainings as mandated by the program.
  • Keep Continuing Education Units (CEUs) up to date.
  • Answer incoming phone calls and handle messages promptly and professionally.
  • Maintain accurate attendance sheets and daily record-keeping.
  • Complete daily menu production sheets and present the current event program to participants daily.
  • Oversee the management of attendant care services, including scheduling and reporting, in accordance with CHFS guidelines.
  • Ensure adequate staffing levels and proper documentation of attendant care services provided.
  • Ensure that all data and service delivery information is recorded accurately within the EVV (Electronic Visit Verification) system, as required by CHFS.
  • Adhere to the guidelines and protocols established for EVV documentation to maintain compliance with regulatory requirements.
  • Provide additional healthcare related services within the Foothills Health and Wellness Center, under the direction of a provider, as needed by the agency.
  • Perform any other duties as assigned by the supervisor or necessary for the smooth functioning of the ADC.
