About The Position

Work this unique schedule and get your 40 hours in just 3 days! Work two doubles - 6a-2p/2p-10:30p 2 days a week and one 2p-10:30p shift EOW. This is a full-time position with full benefits! Luther Park Community has provided quality services for mature Iowans meeting their physical, spiritual, social and psychological needs in its beautifully landscaped community in Des Moines since 1914. As a non-profit, we are able to put our resources into providing better service, care and housing for our residents. The Trinity Center is a 120-bed skilled nursing facility which specializes in Alzheimer’s care. If you are a skilled, compassionate nurse that is dedicated to making sure the best quality of care is given, we want to talk to you! We are looking for an RN/LPN with prior experience in long-term or post-acute care with solid clinical and leadership skills with the ability to work independently to work two 16 hour shifts a week from 2p-10p/10p-6:30a and an 8 hour shift from 2p-10:30p EOW. JOB SUMMARY The Charge Nurse will be responsible for planning, organizing, and carrying out nursing care of residents; orientation, instruction and supervision of CNA, CMA and resident care specialists; carry out or assign nursing duties to other personnel and comply with established policies and procedures and perform professional nursing duties within scope of licensure

Requirements

  • Graduate of an approved school of nursing for RN/LPN
  • Current nursing license in good standing in the state of Iowa
  • Current CPR certification preferred or obtained within 30 days of hire
  • One year of experience in a geriatric setting preferred
  • Knowledgeable with current changes in nursing practice
  • Strong computer skills including knowledge of electronic medical records required
  • Excellent written and oral communication skills
  • Background check, physical, drug screen and TB test required

Nice To Haves

  • Current CPR certification preferred or obtained within 30 days of hire
  • One year of experience in a geriatric setting preferred

Responsibilities

  • Keep DON/ADON informed of resident status and other pertinent matters
  • Admit and orientate new resident and involves family with orientation
  • Check physician orders, transfer forms and pre-admission information
  • Review resident's medical and social history
  • Complete admission and documentation forms
  • Ensure resident's supplies and medications are ordered and equipment is working properly
  • Maintain proper documentation of facility records: narcotic records, medical chart forms, weekly skin assessment
  • Maintain daily, weekly or monthly weight and vital signs and informs physician, DON, ADON and On Call Manager of any changes
  • Complete incident reports, investigation reports and notify physician, family members, DON/ADON and On Call Manager
  • Discharge residents to community or other facilities, transfers residents to hospital, completes required forms and documentation, ensures resident is escorted to transportation and contact family prior to any transfers to hospital
