Requirements

Current licensure by Board of Registration in Nursing (LPN or RN) is required.
One to three years of nursing experience is required.
At least two years of experience in an SNF/Sub acute care facility is required.
Must possess and demonstrate highly effective interpersonal, customer service, and communication skills.
Requires proficient computer skills, including experience with various software, database, and electronic records programs.
Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.

Nice To Haves

Preferable in an administrative / admissions role.

Responsibilities

Communicate with Skilled Nursing Facilities for bed offers for Southcoast Hospitals.
Communicate with case managers in SLH, CMH, and Tobey regarding bed offers from SNFs for patient placement from acute care to SNFs.
Manage the referral to bed offer process from start to finish.
Manage information flow regarding patient needs and issues related to the patient discharge process, scheduling, and insurance issues.
Build and maintain strong relationships and rapport with SNF accounts to secure bed offers for timely patient transfers from acute care.
Document bed offers, bed flows, and discharge calendars in each SNF to raise awareness about access to care.
