About The Position

Family First Homecare is hiring compassionate RNs and LPNs for multiple urgent pediatric home care cases across Polk County. High need for trach/vent nurses. Day, night, and weekend shifts available. The role involves providing high-acuity (trach/vent/complex care) and moderate-acuity (g-tube/seizures/oxygen/mobility) nursing care to pediatric patients in their homes. Requirements

  • Active Florida RN or LPN license
  • Pediatric or home care experience preferred
  • Trach/Vent experience required for high-acuity cases
  • Reliable, consistent, and patient-focused
  • LPN/LVN Nursing Degree.
  • Unencumbered LPN/LVN License in the state of employment or Multi-State license through the Enhanced Nurse Licensure Compact where recognized.
  • 1 years of demonstrated experience
  • Valid driver license with proof of insurance in applicable states
  • Clear background screening
  • Working knowledge of Nurse Practice Acts
  • Valid and current CPR card/certificate with in person instructor led hands on skills check
  • Clear Hep B Screening in applicable states
  • Clear TB Screening in applicable states
  • The ability to read and communicate in English both verbally and in writing.

Responsibilities

  • Providing care that is based on licensure/certification within the scope of practice.
  • Maintaining an updated clinical record on each patient at all times, meeting required deadlines for documentation.
  • Evaluating the effectiveness of nursing service to the patient and family on an ongoing basis.
  • Consulting with physicians concerning alterations of patient care plans, documentation of changes in patient care and reports all changes in care to the nurse supervisor.
  • Mentoring and/or coaching new employees during patient specific orientations.
  • Educating patients and their caregivers on topics related to patients' care needs.
  • Participating in FFHC Quality Assurance, Performance Improvement program (QAPI) as requested.
  • Continually striving to improve his/her nursing care skills.
  • Attending and/or completing all mandatory training/ in-services as assigned.
  • Submitting required personnel file documentation in a timely manner.
  • Adhering to FFHC compliance and ethics expectations.
  • Maintaining patient and family confidentiality.
  • Maintaining professional, supportive, and responsive interpersonal communication skills.
  • Performing other duties as assigned.
  • Delivering high-quality, in-home nursing care to pediatric patients.
  • Providing comprehensive nursing services, adhering strictly to Family First Homecare policies, state Nurse Practice Acts, and all applicable State and Federal regulations.
