About The Position

Care Around the Block helps older adults and their families navigate the challenges of aging with confidence, dignity, and personalized support. We provide care management, advocacy, and coordination services that empower clients to live safely and independently while giving families peace of mind. Care Around the Block is seeking a compassionate, organized, and self-motivated Care Manager to join our team on a part-time basis. This role offers a unique blend of meaningful client work and professional autonomy. Care Managers independently manage their caseloads, scheduling client visits, appointments, and communications based on client needs and their own availability. Rather than working a traditional fixed schedule, Care Managers have the flexibility to build a workload that fits their lifestyle while maintaining responsive, high-quality service for clients and families. This position is ideal for experienced professionals who enjoy working independently, managing their own time, and making a meaningful difference in the lives of older adults.

Requirements

  • Active Nursing license required, RN preferred
  • Strong assessment, communication, and problem-solving skills
  • Ability to work independently and manage a flexible caseload
  • Excellent organizational and time-management abilities
  • Reliable transportation and willingness to travel throughout Knoxville and the surrounding areas
  • Proficiency with basic technology and electronic documentation
  • Professional, compassionate, and client-centered approach

Nice To Haves

  • Experience working with older adults, family caregivers, healthcare systems, or care coordination preferred, but not required

Responsibilities

  • Conduct comprehensive client assessments
  • Develop and implement individualized care plans
  • Coordinate healthcare, home care, community, and support services
  • Advocate for clients during healthcare appointments and care transitions
  • Maintain ongoing communication with clients, families, and providers
  • Monitor client well-being and adjust care plans as needed
  • Respond to client concerns and emerging needs in a timely manner
  • Maintain accurate and professional documentation
  • Participate in team collaboration and case consultation
