About The Position

The Care Team Navigator will ensure that Ohio’s Hospice patients and families receive excellent care and services. The Care Transition Navigator will coordinate care and provide support to patients, families, and Ohio’s Hospice teammates.

Requirements

  • RN’s - Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred
  • LPN's - Registered LPN with the State of Ohio without any board actions.
  • One-year nursing experience preferred.
  • Active Registered Nurse license in the State of Ohio without any board actions.
  • Ability to provide direct patient care
  • Two years acute care nursing experience preferred
  • Certification in Hospice & Palliative Medicine (CHPN) preferred and will be made available to Ohio’s Hospice employees who wish to attain certification
  • CPR Certified
  • Computer skills sufficient to properly document services and care.
  • Ability to drive during daytime, nighttime or inclement weather.
  • Valid Driver's License with Safe Driving Record
  • State Minimum Automobile Insurance Coverage

Nice To Haves

  • Bachelor of Science in Nursing (BSN) preferred
  • One-year nursing experience preferred.
  • Two years acute care nursing experience preferred
  • Certification in Hospice & Palliative Medicine (CHPN) preferred and will be made available to Ohio’s Hospice employees who wish to attain certification
