About The Position

This role is for a compassionate Registered Nurse passionate about helping individuals live independently in their communities. The position involves supporting individuals with long-term care needs through person-centered, consumer-directed services. In this dynamic role, you will assess, plan, and coordinate in-home support services that promote independence, dignity, and quality of life. You'll collaborate closely with consumers, caregivers, and community partners to ensure individuals receive the care and resources they need, while empowering them to stay in control of their care. This position is ideal for nurses who are ready to step away from traditional bedside care but still want to make a meaningful impact, offering no weekends or overnight shifts, flexible scheduling, less direct patient care, and the opportunity to use clinical expertise in a new, rewarding way.

Requirements

  • Registered Nurse (RN) with a current New Hampshire license
  • Minimum of 2 years' experience working with individuals with long-term health care needs
  • A strong commitment to supporting independence and self-direction for individuals with disabilities or chronic health conditions
  • Excellent communication, organizational, and collaboration skills

Responsibilities

  • Conduct assessments to determine eligibility for PCA and other in-home support services
  • Develop, review, and adjust individualized care plans to ensure evolving needs are met
  • Coordinate services and connect consumers to community resources that enhance independence
  • Provide training and education to caregivers (ACEs) to ensure success in their roles
  • Empower consumers through skills training focused on independence and self-direction
  • Support hiring processes for care attendants, including application guidance and onboarding
  • Monitor care plans quarterly (or more frequently as required), including 6-month and annual reassessments
  • Address healthcare management issues and participate in risk assessments when needed
  • Advocate for consumers by collaborating with MCO care managers and case management teams
  • Serve as a resource on personal care services, disability supports, and community programs
  • Conduct training sessions for staff and caregivers
  • Assist with outreach efforts and promote awareness of consumer-directed care options
  • Maintain accurate, up-to-date records in compliance with GSIL policies and procedures
  • Ensure all documentation meets regulatory and organizational standards
  • Manage a caseload of PCA, NF Transition, and/or PCSP consumers
  • Assist with Nursing Facility Transitions and ongoing monitoring
  • Support ACE recruitment efforts and maintain accurate requests
  • Participate in committees, trainings, and professional development opportunities
  • Work a flexible schedule in accordance with program and consumer needs
  • Perform other duties that support independent living opportunities
