About The Position

Lakewood Health System is a comprehensive rural healthcare system in central Minnesota, dedicated to providing quality, personalized healthcare. We are seeking a Registered Nurse (RN) for our Care Center, a 87-bed, Medicare-certified skilled long-term care facility. This role offers the opportunity to build relationships with residents and their families, providing care across long-term care, a specialized dementia unit, and a transitional care unit. You will help residents maintain their abilities, receive personalized care, and feel respected in their home. This position is ideal for nurses looking to strengthen their clinical judgment, develop leadership skills, and work collaboratively with a dedicated team.

Requirements

  • Graduation from an accredited nursing school.
  • Current Minnesota RN licensure.
  • BLS Healthcare Provider certification.
  • Completion of continuing education required to maintain RN licensure.

Responsibilities

  • Assess resident needs, recognize changes in condition, and respond to situations requiring RN intervention.
  • Guide care decisions and update individualized care plans to reflect residents’ needs and preferences.
  • Administer medications and complete treatments with careful attention to safety and accuracy.
  • Support restorative nursing programs that help residents maintain function and independence.
  • Communicate changes in condition with families and collaborate with providers and interdisciplinary partners, including during hospice and palliative care discussions.
  • Guide LPNs, medication aides, and resident assistants through clear expectations, education, feedback, and accountability.
  • Complete timely documentation and thorough shift handoffs so the next team has the information needed to continue care.
  • Participate in quality, physician, and pharmacy rounds and complete follow-up actions.
  • Follow infection prevention practices, safety standards, and care requirements.
