About The Position

Navitas Healthcare, LLC is seeking an experienced RN – Long Term Care (LTC RN) to provide compassionate and individualized nursing care within a rehabilitation and nursing facility setting. The RN will assess, plan, implement, and evaluate patient care while supporting residents with memory care, rehabilitation, skilled nursing, and chronic health conditions. The ideal candidate will possess strong clinical judgment, effective communication skills, and the ability to work efficiently in a long-term care environment.

Requirements

• Minimum 1+ year LTC experience required
• Candidates with recent rehab, med-surg, or hospital experience will be considered
• Strong communication and patient care skills
• Ability to work effectively in a long-term care setting
• Active New York RN License required
• BLS Certification required
• Proof of NY License required at time of submission
• No license hits allowed

Nice To Haves

• Experience caring for dementia and Alzheimer’s patients preferred
• First-time travelers accepted
• BSN preferred

Responsibilities

• Assess, plan, implement, and evaluate individualized resident care plans
• Provide nursing care for long-term care and rehabilitation residents
• Care for residents with Alzheimer’s and dementia-related conditions
• Monitor patient conditions and communicate changes to healthcare providers
• Administer treatments and perform nursing interventions as ordered
• Document patient care accurately within facility standards
• Collaborate with interdisciplinary healthcare teams to coordinate care
• Provide patient and family education regarding care plans and treatments
• Maintain infection control and patient safety standards
• Support resident comfort, dignity, and quality of life initiatives
