About The Position

Reporting to the Manager of Hospice Coordination, the RN Liaison/Admission facilitates referrals to hospice, assists in developing relationships with business partners in the community, and performs timely, individualized, accurate assessments for patients referred to the organization. Also, this position is responsible for performing timely, individualized, accurate assessments for patients referred to Gulfside Healthcare Service for Hospice services as they relate to the Local Coverage Determinations. This Gulfside Healthcare Services position requires an AHCA Level 2 Fingerprint screening for eligibility through the AHCA Florida Care Provider Background Screening Clearinghouse.
