RN Liaison - Inpatient Rehab (Per Diem)
About The Position
The RN Liaison acts as a coordinator between internal and external referral sources, and the acute inpatient rehabilitation operations. The RN Liaison is responsible for establishing relationships with case coordinators, physicians, payors, and other referral sources. The RN Liaison will ensure appropriate patient placement through clinical screenings and assessments in collaboration with both hospital Case Coordination and the Inpatient Rehabilitation Medical Director. The RN Liaison works closely with hospital Case Coordinators and the acute inpatient rehabilitation admissions team to ensure a responsive and smooth flowing referral and admission process.
Requirements
- Current CT RN License
- BSN required
- 3 years recent clinical experience in an acute care setting
Nice To Haves
- Previous experience in a physical rehabilitation setting preferred
