About The Position

This position is responsible for overseeing activities within the team in collaboration with the Manager/Director. The Lead Wound Care Specialist provides care to medically complex patients in collaboration with the physician in a specialized acute care setting. This role works autonomously and has authority, responsibility, and accountability for the assessment, planning, implementation, and evaluation of the wound, ostomy, and continence care program throughout the inpatient care areas. The specialist serves as a resource for wound care management, including identification, assessment, treatment, and prevention. They provide specialized consultation on wound, ostomy, and continence issues for patients, families, nursing, and medical staff. Additionally, the role serves as a consultant in the management of patients with draining wounds, fistulas, and tubes, as well as for the treatment of patients with altered skin integrity due to pressure necrosis, drainage, or incontinence. The specialist works with physicians and nursing staff to provide updated information regarding wound care and conducts daily rounds on all patients with wound/pressure ulcers as needed.

Requirements

  • Registered Nurse (RN) licensure in the state of practice: Required
  • Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy: Required
  • Basic Life Support (BLS) certification or Cardiopulmonary Resuscitation (CPR) certification from approved vendor per AH policy: Required
  • Hospital Fire and Life Safety (HLFS): Required

Nice To Haves

  • Bachelor's Degree (BSN): Preferred
  • Relevant clinical nursing experience: Preferred
  • Wound care certification: Preferred with plan to achieve within 12 months of beginning certification program

Responsibilities

  • Works with physicians and nursing staff to provide updated information regarding wound care.
  • Conducts rounds on all patients with wound/pressure ulcers on a daily and as needed basis.
  • Provides feedback to associates to help develop necessary skills.
  • Coordinates plans to accomplish team goals and collaborates with education and training to provide education specific to their specialty.
  • Disseminates important information to team members and travels to both sites as needed to provide direct patient care and/or team lead responsibilities.
  • Collects relevant data pertinent to the patient's health or situation.
  • Analyzes assessment data in determining diagnosis and care issues.
  • Develops a plan that prescribes interventions to attain outcomes.
  • Implements the plan, coordinates care delivery, and employs strategies to promote health and a safe environment.
  • Evaluates progress toward attaining outcomes.
  • Identifies outcomes for the patient or the patient's situation.
  • Collaborates with the team of patient, family, and healthcare providers in providing patient care in a safe, healing, humane, and caring environment.
  • Provides learning opportunities for patients/family members and team members.
  • Directly provides health information to patients, families, and treatment team.
  • Maintains appropriate supply of equipment and materials.
  • Maintains complete, accurate, concise, legible documentation of patient status, interventions, and responses.
  • Performs other job-related duties as assigned.
