Clinical Transplant Services provides specialized care for transplant patients throughout all phases of the transplant journey, including evaluation, pre-transplant preparation, transplant hospitalization, and post-transplant follow-up. The department consists of four transplant programs: Kidney/Pancreas, Liver, Heart, and Pediatric Kidney. Each program works closely with multidisciplinary teams to coordinate care. Our goal is to deliver high-quality, patient-centered care while promoting positive outcomes and the efficient use of healthcare resources. RN Transplant Coordinators provide comprehensive care coordination and clinical support for transplant patients throughout all phases of the transplant journey. They assess patient readiness, provide education, coordinate diagnostic testing and treatments, monitor clinical progress, and serve as a primary point of contact for patients, families, and the multidisciplinary transplant team. RN Transplant Coordinators collaborate closely with physicians, advanced practice providers, social workers, nutritionists, pharmacists, financial coordinators, community providers, and other healthcare professionals to ensure timely, patient-centered care. They facilitate care planning, coordinate transitions across care settings, and help address clinical and psychosocial needs that may affect transplant outcomes. RN Transplant Coordinators support quality patient outcomes and process improvement initiatives by participating in multidisciplinary team meetings and collaborative transplant care activities and are an integral part of the transplant care team. Communication methods include telephone, voicemail, Microsoft Teams, Epic, and email. Able to triage and prioritize patient issues using critical thinking skills. Excellent written and verbal (incl. face-to-face) communication skills. Excellent organizational and time management skills with attention to detail. Able to work as part of a team and to work independently and problem-solve. Able to work and completed assignments with frequent interruptions.
