About The Position
Responsible for accepting, processing, and coordinating all referrals for patient care services. Assures quality patient care by receiving and analyzing each intake request for clinical and financial appropriateness. Oversees processes for insurance verification and billing accuracy. Receives and processes intake data from referral sources adhering to compliance requirements of CMS and other payers. Coordinates all patient services in coordination with the Director of Intake, Clinical Services and/or Case Managers.
Requirements
- Current and active RN license in the state of employment.
- Minimum of 1 year of experience in home health and/or hospice care.
- Knowledge of home health and/or hospice eligibility.
- Proficient in Microsoft Office and various other programs and systems.
- Exceptional communication and interpersonal skills.
- Ability to work independently and as part of a team.
Nice To Haves
- Bachelor’s degree in nursing (BSN) preferred.
