About The Position

The Heart Failure Integrated Practice Unit (IPU) serves as the foundation of a nationally recognized heart failure program, grounded in clinical excellence, innovation, and patient-centered care. Our program has achieved Heart Failure Certification through DNV and Gold Plus Status from the American Heart Association, reflecting our commitment to evidence-based standards and continuous improvement. The multidisciplinary team advances patient outcomes through comprehensive education, individualized transition planning, and long-term disease management, impacting reduced readmissions, improved symptom control, and enhanced quality of life. The heart failure nurse is central to this work, providing expert nursing care by utilizing the nursing process to assess, plan, implement, and evaluate patient-centered care across the continuum. Practicing within the framework of organizational policies and procedures, the heart failure nurse demonstrates professional accountability, clinical leadership, and a commitment to ongoing growth. This role is responsible for upholding standards of nursing practice, coordinating patient care activities, and collaborating with the interdisciplinary team to ensure safe, effective, and high-quality care. Through clinical expertise, coordination of care, and patient advocacy, the heart failure nurse contributes significantly to the success of the program and the delivery of exceptional outcomes for our patients.

Requirements

  • Graduate from a recognized, accredited school of nursing
  • No experience required
  • New Graduates of a nursing program eligible
  • RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure
  • Current License in the State of Georgia as a Registered Nurse or NLC/eNLC Multistate License.
  • BCLS - Basic Life Support

Nice To Haves

  • Bachelor’s Degree
  • Nursing Experience in Hospital Setting
  • 1 year in a hospital setting for PRN positions
  • Advanced certification in field of specialty, if applicable (see addendum)

Responsibilities

  • The staff nurse provides nursing care to patients from birth through the lifecycle utilizing nursing processes to assess, plan, implement, and evaluate the care for patients.
  • He/she functions within the framework of the policies and procedures of the organization and demonstrates professional growth and accountability.
  • The staff nurse is responsible for maintaining standards of practice, coordinating patient care activities of all assigned staff in the provision of quality nursing care.
