About The Position

Sea Mar Community Health Centers is seeking an On Call Registered Nurse (RN II) to join their team. This position is responsible for coordinating pre and postnatal risk assessment, teaching, and appropriate referral for Sea Mar OB clients and eligible First Steps clients. The role involves conducting nursing assessments, providing education on various aspects of pregnancy and childbirth, and making referrals to necessary services. The RN will perform early postpartum home or office visits, triage client needs, and schedule visits, collaborating with the Sea Mar MSS team and external providers as needed. Active participation in community-wide efforts to improve care for pre and postnatal clients is expected. The productivity expectation is 4-5 patient encounters per day. Responsibilities

  • Coordinating pre and postnatal risk assessment, teaching, and appropriate referral for Sea Mar OB clients and eligible First Steps clients.
  • Providing education on fetal development, normal changes in pregnancy, warning signs, importance of medical care, sexuality/reproductive health, nutrition, activity/exercise, sleep/rest, self-perception, role/relationship, coping/stress, values/beliefs, family planning, breastfeeding, and childbirth preparation.
  • Completing thorough nursing assessments and making referrals to nutrition, social work, mental health, community service agencies, and/or medical providers.
  • Continuing assessments, health messages, and education throughout pregnancy with a minimum of one face-to-face visit per trimester and postpartum.
  • Performing early postpartum home visits or office visits.
  • Triaging pre and postnatal needs of each client and scheduling visits.
  • Coordinating with the Sea Mar MSS team and potentially non-Sea Mar providers.
  • Actively participating in community-wide efforts/coalitions to improve comprehensive interdisciplinary care for pre and postnatal clients.
  • Meeting a productivity expectation of 4-5 patient encounters per day.
