As a professional nurse committed to patient- and family-centered care, you will utilize the nursing process to value the uniqueness of each individual and address the physical, psychological, emotional, and social needs of diverse patients served by Jefferson Health. You will develop nursing plans of care in collaboration with patients and their designated care partners to effectively address individualized needs and priorities, consistent with their cultural beliefs and values. This role involves interacting with colleagues, patients, families, and other staff in alignment with Jefferson's values. You will integrate Evidence-Based Practice, clinical expertise, and patient/family preferences for optimal healthcare delivery. Partnering with patients and families, you will identify learning needs and provide education in an understandable and affirming manner. Minimizing risk of harm through established nursing practices and protocols is essential. You will use information technology for communication, knowledge management, error mitigation, and ethical decision-making. Data will be used to monitor outcomes and drive continuous improvement in patient care quality and safety. Functioning effectively within nursing and inter-professional teams, you will foster open communication, mutual respect, and shared decision-making. Recognizing patients and their families in providing safe, compassionate, and coordinated care based on respect for personal preferences, priorities, needs, and cultural beliefs is key. You will create an environment of belonging and inclusion, demonstrating skills in problem-solving and conflict resolution. A commitment to learning and professional growth is expected, including acting in a primary preceptor role. Active participation in hospital and practice setting-based committees, including safety and quality improvement, is encouraged. Demonstrating willingness to give and receive feedback and integrate appropriate changes into practice is also important.
