About The Position

Cleveland Clinic’s Business Operations Center serves as a key hub for our Home Care and Hospice operations as well as many administrative and support teams. This collaborative workplace supports a wide range of caregivers, clinical professionals, and operational teams. While many positions are based within the center, numerous home-visiting roles—including case managers, nurses, and therapists—travel throughout the Greater Cleveland and Akron/Canton areas to deliver exceptional care directly to patients in their homes. Hospice helps patients and their families prepare physically, emotionally and spiritually for the end of life. Our focus is on providing comfort, not a cure, and on quality of life, not duration. As RN this team, you will collaborate with physicians, RNs, Social Workers, Home Health Aides and volunteers to support patients wherever they call home. This position offers a flexible schedule and autonomy to work independently, allowing you to maintain a healthy work-life balance. This position will provide care to patients living within the Akron/Canton and surrounding areas. A caregiver in this position works 11:00am to 11:30pm, 3 days out of the week on 12-hr shifts, with weekend rotations occurring every 3 weeks. 1 winter and 1 summer holiday is required.

Requirements

  • Graduate from an accredited school of Nursing
  • Current licensure as a Registered Nurse (RN) in the State of Ohio
  • Basic Life Support (BLS) Certification from the American Heart Association (AHA)
  • One year of current clinical experience as an RN
  • One year of experience in an acute medical-surgical or critical care setting
  • Valid Driver’s License (VDL) and proof of automobile insurance with 100,000/300,000 coverage required

Nice To Haves

  • Bachelor’s of Science in Nursing (BSN)
  • Hospice and Palliative Care Nurse Certification

Responsibilities

  • Provide supervision, assessment, planning and direct patient care to hospice patients.
  • Perform accurate and complete admissions assessments and re-assessments.
  • Assess patients for safety risks, abuse and neglect.
  • Assess, document and manage patient’s pain.
  • Prioritize and document patient problems.
  • Administer medications and treatments safely and accurately with a focus on patient comfort.
  • Coordinate care for managing a caseload of patients.
  • Document changes in patient condition and effectiveness and timeliness of care/interventions.
  • Evaluate, update and revise plans of care.
  • Assess educational needs of patients, caregivers and family while considering cultural issues and other special challenges.
  • Provide teaching and educational material to patients and families concerning diagnosis, diagnostic tests and procedures, safe and proper use of equipment, reason for and safe use of medication food/drug interactions, injury prevention, self-care and nutritional needs.
